— My Health Story Premium subscription costs $49 and helps you record, store, share and sync health information online and/or on handheld devices for one year.

— My Health Story Lite subscription costs $29 and helps you more simply record, store, share and sync health information on handheld devices for one year.

Register online or navigate to your App Store to subscribe.

About My Health Story

The My Health Story system

My Health Story Premium subscribers get full access and the ability to sync health information online and across handheld devices.

My Health Story Lite subscribers have access to a smaller menu and manage health information on handheld devices only.

This Profile area is for specifying the following information:

— Impairment status
— Medical Conditions
— Health story visibility

The Body Map function offers a way to indicate and describe health issues:

The Characteristics area provides a way to describe the following:

— How the issue impacts throughout the day
— The daily pattern of the issue

 

Clickable ‘Tags’ and ‘Terms’ are available to help:

— Identify medical terms used to describe treatment and the health experience

 

The MHS system uses Tags and Terms to customise the health references emailed to you and to ensure that only relevant health resources are sent to your inbox.

 

MHS Premium & Lite.

The Daily Living offers a customisable and continuously editable questionnaire to help describe:

— What activities the issue prevents you from doing
— What the painful facts are
— The impact of the issue
— Your physical feelings
— How you feel emotionally
— Support you have
— What exacerbates the issue

— Set goals
