A: A My Health Story account is automatically created upon a paid subscription. This account information is managed by your App Store if you subscribe via the App Store – so only your name and contact email address will be viewable to the MHS system. If you register online, the contact details you enter when registering, such as a phone number, email and postal address will be viewable to the MHS system. My Health Story does not share this information and only uses it for administration and communicating helpful resources. Subscribers can opt out from informational communications at anytime.

A: A health story is only created after a valid subscription has been paid and contains information that a subscriber enters. View an example here.

A: Anyone can subscribe and create a My Health Story account.

A: Anyone can become a My Health Story Friend/Affiliate. View the information here.

A: The subscriber controls who sees their health information by creating Access Links and personally sharing the unique URL. The Access Links are fully controlled by the subscriber and can be revoked at any time.

Anyone with whom the subscriber has shared an Access Link can view the health story for free. Subscribers can also show their information from their device or print their health story also.

A: The health story can be viewed on any browser with a live internet connection. Any updates made by the subscriber sync in real-time. Subscribers can edit and view their health stories anytime.

A: After creating your health story, navigate to the Access menu item and follow the instructions. A subscriber can make various Access Links for various people/professionals.

If you change your mind about an Access Link you created, you can retract/delete the Access Link/s at any anytime. You can email the Access Link to share your health story or print it out to mail or hand out.

A subscriber can also present their health story via a hand-held device.

A: My Health Story Premium allows further privacy control within each section should a subscriber not want to share the full details of the health story/stories. Scroll to the bottom of each section to find this feature which is available in most areas.

A: Only subscribers can make changes to their own patient health story.

A: Yes. Each health story can only be accessed via the patient’s personal and secure login. All users, upon registration, must read and accept the Terms and Conditions.

A: Never. And if ever the situation arises where we feel privacy has been breached, all users will immediately be notified.

A: No, there is no additional cost to the user after the subscription. Unlimited health stories and unlimited access links can be created.

A: Electronic files (such as PDF, jpg, png, doc) can be uploaded and stored securely in the Filing Cabinet of a subscriber’s health story. This information is protected and encrypted during transit. Please read our Disclaimer and Terms and Conditions before uploading your health information.

A: My Health Story protects your data by implementing robust security measures and remaining compliant with data privacy laws. All files are stored on the most trusted WordPress platform WP Engine that encrypts access to the MHS website and database. MHS administrators monitor all admin logins 24/7 and all data transfer is done via SSL Certificate.

A: My Health Story takes every precaution to protect this website and all the information. However, like all information submitted on the internet, there are risks of theft.

To avoid using your identity:
  • Set up a private email address with your existing provider that does not identify you (Here are some email providers and information on how to go about setting up your service)
    • Use a private email address and subscribe to MHS Premium online via a private browser window
    • Enter random contact details in the form and then download the App from your App Store.
    • Log in with the username and password you used to purchase the subscription online.

A: My Health Story doesn’t archive your personal health information; therefore, the full health story will be deleted after you have been notified.
